Tribute Request Information
Tribute requests can be made using the dropdown on the Request a Tribute tab or by clicking the buttons below.
Please provide the following information:
- Deceased nurse’s name
- Funeral Home handling arrangements
- Funeral Director in charge
- Family/friend contact after family permission for tribute has been obtained
- Family contact phone number
A CNHG Tribute Coordinator will be in contact with the Funeral Home and family within 24 hours of notification. A 48 hour or longer lead time is much appreciated.
The Tribute can be provided during final minutes of visitation, during a funeral or memorial service, celebration of life or at graveside – per the family’s wishes.
A Living Tribute is a heartfelt ceremony presented while the nurse is still living, often in hospice or late-life care. It allows them to hear words of gratitude and appreciation for their lifetime of service.
A Tribute for a Deceased Nurse is performed during or around memorial arrangements to honor and recognize their career and contributions after their passing.
